ขับเคลื่อนเชิงนโยบายระบบการดูเเลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้าน

Abstract

โครงการวิจัย "การขับเคลื่อนเชิงนโยบายระบบการดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้าน" เป็นโครงการวิจัยระยะที่ 2 ซึ่งพัฒนาต่อเนื่องมาจากโครงการวิจัยการพัฒนาระบบการดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยไร้บ้านในพื้นที่กรุงเทพมหานคร โดยมีวัตถุประสงค์คือ 1) เพื่อผลักดันนโยบายระบบการดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้านในพื้นที่กรุงเทพมหานคร 2) เพื่อหนุนเสริมเครือข่ายภาคีความร่วมมือ ในการนำนโยบายระบบการดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้านในพื้นที่กรุงเทพมหานครสู่การปฏิบัติ และ 3) เพื่อพัฒนาข้อเสนอเชิงนโยบายระบบการดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้านในระดับประเทศที่ยั่งยืนต่อหน่วยงานที่เกี่ยวข้อง โครงการวิจัยนี้ใช้การศึกษาวิจัยเชิงคุณภาพ โดยผลการศึกษาพบว่า มีปัญหาและอุปสรรคในการดำเนินการของหน่วยงานต่าง ๆ หลายประการ ซึ่งมีผลทำให้ระบบการช่วยเหลือดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้านในพื้นที่ของกรุงเทพฯ ขาดประสิทธิภาพ ดังนั้นจึงต้องมีการผลักดันให้ภาครัฐมีส่วนเกี่ยวข้องในการกำหนดนโยบายอย่างยั่งยืนและเป็นรูปธรรมในการดูแลให้ความช่วยเหลือแก่ผู้ป่วยจิตเวชไร้บ้าน โดยจะต้องมีการดำเนินการครอบคลุมทั้ง 3 ระบบ ได้แก่ ระบบนำเข้า ระบบรักษา และระบบจำหน่ายหรือส่งต่อ ทางผู้วิจัยจึงมีข้อเสนอแนะให้มีการดำเนินการต่าง ๆ ได้แก่ การจัดทำบันทึกข้อตกลงความร่วมมือระบบการดูแลผู้ป่วยจิตเวชไร้บ้านในเขตพื้นที่กรุงเทพมหานครระหว่างหน่วยงานภาคีเครือข่าย, การจัดตั้งคณะกรรมการขับเคลื่อนระบบการดูแลให้บริการด้านสุขภาพจิตแก่ผู้ป่วยจิตเวชไร้บ้าน โดยกรมสุขภาพจิตกระทรวงสาธารณสุข, การทดลองจัดทำฐานข้อมูลกลาง (Data base center) ของผู้ป่วยจิตเวชไร้บ้าน โดยความร่วมมือของโรงพยาบาลพยาบาลจิตเวช 3 แห่ง ได้แก่ สถาบันจิตเวชศาสตร์ สมเด็จเจ้าพระยาโรงพยาบาลศรีธัญญา และสถาบันกัลยาณ์ราชนครินทร์ เพื่อการดูแลรักษา การสืบหาญาติและภูมิลำเนาที่อยู่อาศัย, การจัดทำคู่มือการนำส่งผู้ป่วยไร้บ้านที่มีปัญหาด้านสุขภาพจิตสำหรับตำรวจ พนักงานฝ่ายปกครองหรือผู้ปฏิบัติตามกฎหมายว่าด้วยการแพทย์ฉุกเฉิน และการจัดทำคู่มือการฟื้นฟูสมรรถภาพทางจิตเวชของผู้รับบริการในสถานคุ้มครองและพัฒนาคนพิการบ้านกึ่งวิถีหญิง เป็นต้น ทั้งนี้เพื่อให้กลุ่มผู้ป่วยจิตเวชไร้บ้านเหล่านี้ได้รับการคุ้มครองดูแลรักษา การช่วยเหลือฟื้นฟูจากรัฐตามสิทธิของตนเอง และสามารถกลับออกไปใช้ชีวิตในครอบครัวและสังคมได้อย่างมีคุณภาพต่อไป The research project namely “Policy Steering of Mental Health Care System for the Homeless Psychiatric Patients” which is the 2nd phrase research project developed from the previous research project of the development of mental health care system for the homeless psychiatric patients in Bangkok area aims at 1) pushing a policy on mental health care system for the homeless psychiatric patients in Bangkok area; 2) supporting the collaboration network to implement the policy on mental health care system for the homeless psychiatric patients in Bangkok area into practice and 3) developing the national sustainable policy suggestions for the mental health care system for the homeless psychiatric patients to related agencies. The research uses qualitative research, resulting findings of several problems and obstacles of various agencies’ operations that cause the mental health care system for the homeless psychiatric patients in Bangkok area becoming inefficient. As a result, related state agencies should be pushed for determination of concrete and sustainable policy on provision of care and assistance to the homeless psychiatric patients by including these following 3 systems i.e. input system, treatment system and distribution or referral system. The researchers suggest these following actions i.e. networked agencies entering into memorandum of cooperation on the care system for the homeless psychiatric patients in Bangkok area, setting up a steering committee on the mental health care system for the homeless psychiatric patients by inclusion of Department of Mental Health, Ministry of Public Health, trial to set up a database center for the homeless psychiatric patients with cooperation of three psychiatric hospitals i.e. the Somdet Chaopraya Institute of Psychiatry, the Srithanya Hospital and the Galyarajanagarindra Institute in benefit of care and treatment, detection for relatives, domicile and living place, production of a manual of escorting the homeless psychiatric patients to be utilized by police, administrative officer or people abide by law on emergency medicine and production of a manual of rehabilitation of psychiatric capacity of service users in the Half Way Home for Women. Eventually, these homeless psychiatric patients will be better cared, protected and rehabilitated with respect of their own rights by the public sector and further restored to leave for quality living with their family and society.
